Spray Tan Consent & Liability Waiver

1. Voluntary Consent

I voluntarily request and consent to receive a sunless spray tanning service from Honeyburst Glow.

I understand that a spray tan is a cosmetic application that temporarily colors the outer layer of my skin and does not provide protection from ultraviolet (UV) radiation or prevent sunburn.

2. Health Disclosure

I certify that I have answered all health and intake questions honestly and to the best of my knowledge.

I understand that it is my responsibility to disclose any condition that could affect my spray tanning service, including but not limited to:

Allergies or sensitivities to cosmetic products
Asthma or respiratory sensitivities
Open wounds, cuts, or skin infections
Sunburn
Eczema, psoriasis, or other skin conditions
Recent laser treatments
Chemical peels
Microneedling
Recent waxing or shaving
Pregnancy or breastfeeding
Any other medical condition that may affect my skin or the application of the tanning solution

3. Individual Results

I understand that spray tanning results vary from person to person.

Honeyburst Glow cannot guarantee:

Results depend on many factors including:

An exact color outcome
A specific shade
Exact longevity
Identical results between appointments

Skin tone
Skin chemistry
Preparation
Aftercare
Medications
Lifestyle
Swimming
Sweating
Exfoliation

4. Preparation & Aftercare

I understand that following the recommended preparation and aftercare instructions is essential for achieving the best results.

Failure to follow these instructions may affect color development, longevity, and evenness of application.

Honeyburst Glow cannot be responsible for diminished results caused by failure to follow recommended preparation or aftercare instructions.

5. Allergic Reactions

Although adverse reactions are uncommon, I understand that cosmetic products may cause allergic reactions or skin sensitivities.

I agree to notify Honeyburst Glow immediately if I experience an unexpected reaction following my appointment.

6. Clothing & Personal Property

I understand that cosmetic bronzer may temporarily transfer to clothing, sheets, towels, or upholstery before my first rinse.

While the cosmetic bronzer is generally washable, Honeyburst Glow cannot guarantee stain removal from all materials and is not responsible for damage to clothing or personal property.

7. No Medical Advice

I understand that Honeyburst Glow does not provide medical advice. If I have concerns regarding pregnancy, allergies, skin disorders, medications, or other medical conditions, I agree to consult my physician before receiving spray tanning services.

8. Limitation of Liability

To the fullest extent permitted by applicable law, I release Honeyburst Glow, LLC, its owners, employees, contractors, and representatives from liability for claims, injuries, damages, allergic reactions, losses, or expenses arising from my voluntary participation in spray tanning services, except where caused by gross negligence or willful misconduct.

Nothing in this agreement is intended to waive rights or protections that cannot legally be waived under applicable law.

9. Photography (Optional)

From time to time, Honeyburst Glow may request permission to photograph spray tanning results. Photographs will only be taken and used with my separate permission. My decision regarding photographs will not affect my ability to receive services.

10. Electronic Consent

By selecting the required consent checkbox during online booking, I acknowledge that:

I have carefully read this Spray Tan Consent & Liability Waiver.
I understand its contents.
I have had the opportunity to ask questions before my appointment.
I voluntarily consent to receive spray tanning services from Honeyburst Glow.
My electronic acknowledgment has the same effect as my handwritten signature for purposes of confirming my agreement to this consent.
